How to Submit a PFML Insurance Leave Request

Qualifying Event
Medical Leave
Caring for the covered individual’s own serious health condition.
Family Leave
Bonding with a new child during the first year or caring for a family member with a serious health condition.
Safe Leave / Personal Protected Leave
Seeking help relating to domestic violence, harassment, bias crimes, sexual assault, or stalking experienced by a covered individual or a minor child/dependent.
Notify Your Employer
30-day notice
If you know you need to take paid leave, inform your employer at least 30 calendar days beforehand, unless giving advance notice is not possible.
24-hour emergency notice
Employees should always give employers as much advance notice as possible before taking leave. In an emergency, you must at least verbally tell your employer that you plan to use paid leave within 24 hours. An additional written notice is required within three days after starting the leave.
Submit Leave Request
Log in to your online account and click Request Leave, then follow the on-screen prompts.
Gather Required Documentation
You are required to submit documentation to support your leave request. This documentation must be complete, accurate and submitted on time, or your leave application may be denied. It is very important that you immediately begin to gather and submit all required documentation. You can view the requested documents in your online account. You will be notified if any additional documents are required.
Review Request
Once we receive your leave request with the required documentation, we’ll begin reviewing it. After receiving all required documentation and information, processing time can take up to 14 calendar days. American Fidelity will contact you directly once your request has been approved or denied.
