• Driver’s license.
• Bank account information (to sign up for direct deposit)
 • Spouse and children’s date of birth and Social Security number if considering coverage for them.
• Beneficiary information, including (if a trust) full name and date of trust.

Maximize Your Income

Your benefits are offered under a Section 125 Plan, which may provide financial advantages. Under this plan, premiums for benefits are taken from your paycheck before tax, allowing you more spendable income.

Your Section 125 benefits don’t automatically renew each year, so be sure to complete your enrollment and sign your election form—even if you don’t plan to make any changes.
